This practice are commissioned by Birmingham and Solihull ICP (formally CCG). The practice is also forms part of i3 PCN.
Accountable/Named GP
From the 1st of April 2015 practices are required under the GMS contract, to allocate a named accountable GP to all patients including children. It is important that you are aware that this is purely an administrative exercise and does not change the way in which we operate or affect your ability to make an appointment or speak with any of the GPs in the Practice.
We have allocated you to the following GP:
ALL men and women aged over 65years, your allocated named GP is Dr I S Marok
ALL children below age 18 years and Females Aged 18 years and 64years,your allocated named GP is Dr Parmjit K Marok
ALL men Aged 18 years and 64years, your allocated named GP is Dr Shabir Mahomed
Comments, Suggestions and Complaints
These are welcome and can be made to the doctors, nurse, receptionists, practice manager or dropped into reception.
Data Protection & Access to Health Records
We fully abide by the rules laid down in these acts and only those persons who have right to see your records will be able to see them.
This practice adheres to all standards of confidentiality. All health professionals, administrative and ancillary staff- including receptionists, secretaries, practice manager, cleaners and maintenance staff who are bound by contracts of employment to maintain confidentiality. They must not reveal, to anybody outside the practice, personal information they learn in the course of their work, or due to their presence in the surgery, without the patient’s consent. Nor will they discuss with colleagues any aspect of a patient’s attendance at the surgery in a way that might allow identification of the patient unless to do so is necessary for the patient’s care.
Who has access to patient information
We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can provide the best possible care. We have a fully computerised medical record system which means information about your healthcare is held on a secure server. You have the right to know what information we hold about you. Please ask the receptionist if you wish to see or obtain a copy of your record. If you are referred to a hospital specialist you are also welcome to request a copy of the referral letter.
Policy on Violent or Abusive Patients
We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we will exercise our right to take action to have them removed, immediately if necessary, from our list of patients.
This practice fully participates with NHS England policy of Zero Tolerance. We report all violent and abusive behaviour directed at the staff or premises to the Police and the offender will be removed from the practice list.
